Medical Requirements for Entry to 6th Grade:
We ask that you submit these by August 1st.
1. IMMUNIZATIONS
All students entering 6th grade must receive one dose of Tdap (Tetanus, Diptheria, Pertussis) on or after their 10th birthday. Record of student receiving this immunization is required.
All students entering 6th grade must receive one dose of a Meningococcal-containing vaccine. Record of student receiving this immunization is required.
2. PHYSICAL EXAMINATION
A physical examination is required for admittance to 6th grade for ALL students:

Required for all incoming students. Pages 1-4 of this document is to be kept on file at your child's doctor's office. Only Page 5 "PPE Medical Eligibility Form" is to be returned to HMS. Physical must have occurred within 365 days of first day of school. (this is also the physical required for student athletes).
PPE Medical Eligibility Form
This is the only physical exam medical form signed by your doctor that should be handed into HMS. This form is also included in the packet linked above (Page 5) titled: Annual Athletic Pre-Participation Physical Examination Form - Complete Packet.
Your child may need additional medical paperwork (ex: allergy or asthma action plan, medication at school, seizure disorder, etc)
